Policies, Insurance & Payments


Payment of fees, including copays

Payment for your session will be due at the time of service. Copays are billed monthly. Please notify me if there are problems during the course of your therapy regarding your ability to make appointments. Defaulted accounts may be sent to collections.

Payment and Insurance claims

I kindly request payment in full at the time care is provided. You may submit claims to your insurance company for reimbursement. If the insurance company sends reimbursement to me in error, I will gladly send a refund to you. I am unable to tell you how much reimbursement you can expect. If you require advance notice of estimated reimbursement by your insurance for a session, you need to contact your insurance company.


Cancellation Policy

Appointments are 55 minutes in length. In the event that you are unable to keep an appointment, please notify me immediately. If you are more than 15 minutes late, are unable to keep an appointment, or miss an appointment without 24 hours’ notice, you are financially responsible for the appointment. Insurers do not pay for missed appointments.

If you need to cancel an appointment, you must notify me 24 hours before our appointment time to avoid paying the full cost of the missed session.

Prior to our first visit, its recommend contacting your insurance company and verifying benefits. It is your responsibility to know your insurance plan.

Phone Calls/Emergencies/Therapist Absences

While I check my messages regularly, I may be unavailable and will return your call when I am able. In the event of an emergency, please call the HELP Center - a local, free, 24-hour crisis line at 586-3333 or 911.

I may be unavailable for regular appointment due to professional or personal obligations. I may arrange for another therapist to be on call in my absence. I may provide identifying information to them to aid in their provision services is needed.
